Bay of Plenty

Te Whatu Ora – Health New Zealand Hauora A Toi Bay of Plenty

Serves Western Bay of Plenty and Eastern Bay of Plenty - Tauranga and Whakatāne.

  • 1. Email us your plan to and it can be uploaded to your health records.

    2. Leave a copy of your plan with your GP it can be uploaded to your health records.

    3. Follow the steps of Manage My Health to upload a copy of your plan and it can be added to your health records

Lakes

Te Whatu Ora – Health New Zealand Lakes

Serves Mourea to Mangakino, Tūrangi across to Kaingaroa village. Major centres - Rotorua and Taupō.

  • Visit your GP to finalise and sign your advance care plan. The general practice team will scan your plan to the hospital system.

Tairāwhiti

Te Whatu Ora – Health New Zealand Tairāwhiti

Serves Tairāwhiti Gisborne.

Taranaki

Te Whatu Ora – Health New Zealand Taranaki

Serves New Plymouth, Taranaki and Hāwera

Whanganui

Te Whatu Ora – Health New Zealand Whanganui

Serves Whanganui city, Marton, Waiouru, Taihape, Bulls, Ohakune and Raetihi.

  • Take a copy to the GP who can then fax, email or post to the hospital. This will then be uploaded to a clinical portal where an alert will be generated.

    Those who do not have a GP can submit to the hospital by emailing cleo.berryman@wdhb.org.nz

Hawke’s Bay

Te Whatu Ora – Health New Zealand Te Pae Hauora o Ruahine o Tararua MidCentral

Serves Horowhenua, Manawatū, Palmerston North, Tararua district and the Ōtaki ward of the Kāpiti coast district.

  • Talk to your GP team about reviewing your advance directives and getting your advance care plan loaded onto their system.

    Please keep an original of your advance care plan and take it with you to health appointments or ED. Remember to keep your advance care plan up-to-date and share any changes with your GP team.

Capital & Coast, Hutt Valley and Wairarapa

Te Whatu Ora – Health New Zealand Capital & Coast, Hutt Valley and Wairarapa

Serves Wellington City, Hutt Valley, Porirua City and the Kapiti Coast at Paraparaumu.

  • Make an appointment with your GP or practice nurse to help you with the medical aspects of your advance care plan.

    Your medical centre can put your signed and dated plan onto your medical centre file and either you or your medical centre can send a scanned copy plan to acp@ccdhb.org.nz to have it added to your hospital file.

    This allows hospital doctors across the region to access and respond to your wishes in an emergency. If you change your plan in the future, share the new version with your medical centre and acp@ccdhb.org.nz

Te Waipounamu – South Island

Nelson Marlborough

Te Whatu Ora – Health New Zealand Nelson Marlborough

Serves Nelson, Tasman and Marlborough regions.

Serves the West Coast of the South Island, covering an area from Karamea in the North to Haast in the South.

Canterbury

Te Whatu Ora – Health New Zealand Waitaha Canterbury

Serves the East Coast of the South Island from Kaikōura in the north, to Ashburton in the south, as well as the Rēkohu/Chatham Islands.
