Changes to advance care planning website

The Health Quality & Safety Commission is coordinating the advance care planning (ACP) programme until the end of 2019, in partnership with district health boards.

The Commission has improved the ACP website (www.advancecareplanning.org.nz) and will be moving the content to the Commission’s website. The ACP website address will not change. You can find all of the same ACP resources and materials in their new location on the Commission’s website.

From 1 July 2018, a redirect will be put in place which will send website traffic from www.advancecareplanning.org.nz to the Commission’s ACP homepage. If your website has any existing links to the old website, these will need to be checked following 1 July. All hyperlinks should only include www.advancecareplanning.org.nz.
