This is the fifth report on serious adverse events published by the Health Quality & Safety Commission, covering events reported by New Zealand's 20 district health boards (DHBs) and other providers, and the eighth since national reporting began.

Serious adverse events (previous referred to as serious and sentinel events) are events which have generally resulted in harm to patients. This report details serious adverse events in DHBs in the year 2013 to 2014.
