The Commission has been working with the SMM team to ensure a smooth transition of the work.
We have had a very good handover, which will help us to maintain momentum and ensure that the implementation dates confirmed with DHBs can be achieved.
A number of people have been seconded or contracted to the Commission to keep key SMM activities on schedule during the transition, and we are recruiting for the following positions: senior portfolio manager, medication quality and safety specialists, project manager, and a project coordinator.
National Medication Chart (NMC) and Medicines Reconciliation (MR)
The key priorities this year are to introduce a standardised national medication chart and a medicine reconciliation process across all DHBs. This work is well in hand.
Eight DHBs have confirmed implementation of the NMC before 1 June 2011: West Coast, Bay of Plenty, MidCentral, Whanganui, Nelson Marlborough, Hawkes Bay, Lakes and Counties Manukau.
With MR, there are only five remaining DHBs to complete the MR implementation goal by the end of December 2011: Bay of Plenty, Canterbury, Waitemata, Hutt Valley and Southern.
