Free Let’s PLAN flyer for consumers available

Don’t forget about Let’s PLAN – the free consumer resource developed by the Commission that encourages people to plan ahead for visits to their GP or other primary health care professional.

Prepare for your visit 
Listen and share
Ask questions
Note down what you need to do next.

The A4 PLAN flyer, co-designed with consumers and health care providers, has space for patients’ questions and notes to help them fully understand their diagnosis and treatment. It also suggests questions they can ask pharmacy staff when they pick up their medicine.

While the intended audience was general practice and community pharmacies, Let’s PLAN has also gained traction with district health boards (DHBs). For example, Taranaki DHB is using it in a community mental health service users’ brochure, Auckland DHB has incorporated it into its discharge packs for every patient, Nelson-Marlborough DHB pharmacy gives it out to all their patients on discharge and Canterbury DHB has used it on their patient portals.

Let’s PLAN will be particularly useful for those who:

  • have multiple or long-term conditions
  • need specialist outpatient appointments
  • live in more isolated areas and so have less frequent interactions with service providers
  • have lower health literacy or who may have more difficulty asking questions of their health care team (for example, including the elderly, Pacific people and migrants and their family/whānau/ and caregivers).

Let’s PLAN was developed as part of the Commission’s Open for better care national patient safety campaign.
