The patient information pack has been designed for help facilitate a conversation with patients (and their families/whānau) about what they can do to reduce their risk of falling while they are in hospital, and when they go home. The pack was developed as a resource for April Falls 2013 activities, and is currently being trialled by Waikato District Health Board (DHB) in a number of inpatient areas.
Three items are packaged in a cellophane wrapper so that the health professional using it doesn’t have to compile the separate resources. We suggest you open it with the patient as you explain its purpose in keeping them safe in hospital and planning for returning home.
One way to ensure patient and family/whānau involvement is to ask them to tick off items in the checklists that apply to them – you can pick up on those they’ve identified (and any they have missed) in a brief conversation.
The pack supports discharge planning, by helping older people and their families think about what they might need to do to make their home safer – there may be steps that family or friends can get started on before the person is discharged.
The Commission is awaiting the findings of Waikato DHB’s trial of the patient information packages before making any revisions to the package.
What is in the patient information package:
The first page gives patients advice on how to stay safe while in hospital, with a checklist of safety tips; the second page contains information on preventing falls at home.
