Auckland DHB

With your consent we can load the plan into Whanau Tahi Shared Care (WTSC).
A copy of your loaded plan will be provided and your GP will be notified.
If the GP does not have access to WTSC we will send an electronic copy to your GP by secure email.
  • Ideally ACPlan’s should be loaded into Whanau Tahi Shared Care (WTSC), which will enable your GP and hospital staff to access your ACPlan.
  • Hard copy ACPlan’s can be mailed to ACP team, Ko Awatea Innovation Hub, Middlemore Hospital, Private Bag 93311, Otahuhu, Auckland 1640
  • eACPlan’s or scanned ACPlan’s can be emailed to AdvanceCarePlanningFacilitator@middlemore.co.nz
Completed ACPlan’s can be taken to your GP, to be loaded into WTSC (a GP fee may apply).
