Learning from your experience

We are sorry if you are reading this because you have lost a child or young person. As a Committee we offer you our condolences and hope you are getting the support you need to deal with this difficult experience.

Our Committee is a statutory committee of the Health Quality & Safety Commission and is mandated to review deaths of children and youth aged 28 days to 25 years. We aim to find ways to prevent such deaths in the future.

If you would like to contribute to our work we would appreciate hearing from you. We would like you to tell us about your experience, as you may be able to help us get a complete picture of how and why some deaths happen. 

How will your information be used?

We will treat any information you provide as confidential and will store it in a secure database.

We review all information gathered about deaths of children and youth in New Zealand and make recommendations to the Commission about how we can reduce preventable deaths. This information is also included in our annual reports. Contact us to receive a copy.
