The aim of Advance Care Planning Day is to explain the benefits of advance care planning and to make it ‘business-as-usual’ for all health care professionals. It also encourages the general public to think about their own future health and wellbeing and what really matters to them.
Two new ACP posters featuring Arthur and Cheryl have been produced with additional space to write your local events.

Orders are generally delivered within two to three weeks of the order being received.
