We are postponing Advance Care Planning Day until later in the year. The day was to be held on Friday 3 April. Read more here.

The aim of Advance Care Planning Day is to explain the benefits of advance care planning and to make it ‘business-as-usual’ for all health care professionals. It also encourages the general public to think about their own future health and wellbeing and what really matters to them.

Two new ACP posters featuring Arthur and Cheryl have been produced with additional space to write your local events.

You can order all ACP resources; the advance care planning guide, the advance care planning brochure and posters here or download PDFs via the relevant pages below.

Orders are generally delivered within two to three weeks of the order being received.

Kia kōrero | Let's talk advance care planning

Kia kōrero | Let’s talk advance care planning is a campaign that encourages people to plan for their future health care. It features the personal stories of six New Zealanders at different stages of life and wellness.

An advance care plan tells your loved ones and health care teams about the treatment and care you want so treatment plans can support what is important to you.
