The always report and review list has been updated for 2018–19, with a minor change to the definition relating to the 'wrong blood component' event. Based on feedback from the NZ Blood Service the definition has been updated to 'actual or near-miss transfusion of ABO incompatible red cells'. The always report and review list is updated regularly by the Health Quality & Safety Commission in consultation with the sector.
