Medicines reconciliation film - NHS employers organisation (UK)
The NHS employers organisation has produced a film about medicines reconciliation to highlight the role each sector plays in medicines reconciliation, particularly hospital pharmacy.

The film goes through the medicines pathway from a patient's point of view, demonstrating that both hospital and community pharmacists can work together to improve patients' understanding of their medicines. Click here to read more about the film.
Medicine reconciliation is an important practice that identifies the most accurate list of a patient’s medicines (including name, dose, frequency, and route) is correct and will enable any discrepancies to be recognised and changes documented, thereby resulting in complete list of medicines. Prescribing errors can result in harm to patients and the aim of medicine reconciliation when patients are admitted to hospital is to ensure that important medicines are not stopped and that new medicines are prescribed.
