Shared goals of care forms for use in hospital

There are three shared goals of care forms. As we wanted to make these forms available as quickly as possible, we have not been able to complete our usual robust quality improvement approach.

These forms are available to all hospitals wanting to use them. They should be used with the shared goals of care factsheets for nurses and allied health workers, response teams and senior clinicians.

The only difference between the three forms is how the rapid response team is referred to: 777, MET or RRT.

They have been designed for in-house printing. If you would like to have a commercial printer option, please contact info@hqsc.govt.nz.

We recommend you use these forms in the context of the other information and resources in the talkingCOVID section of this website.
