Te Rā Haumaru Tūroro o Aotearoa | Aotearoa Patient Safety Day 2022 will be observed on 17 November.

The focus of our activity is on medicines at transitions of care, which ties in with the World Health Organization’s theme for World Patient Safety Day this year: medication without harm.

We have three simple key messages, which we are calling ‘the three Ps’:

  • Provide a complete list of medicines, including indications for the medicines and any changes made.
  • Patient and whānau understand their medicines and any changes.
  • Practicalities of accessing the medicines considered before discharge.

We will be providing pens, two consumer resources and a digital communications pack to health care providers to promote the day.

  • Pens with a pull-out flag containing information about the three Ps.
  • Two consumer resourcesLet’s plan for your next health care visit and Let’s plan to leave hospital – available in a range of languages including English, te reo Māori, Hindi, Simplified Chinese, Samoan, Tongan and the seven other Pacific community languages.
  • Communications kit (electronic) – by the end of October we will email district communications teams a kit that includes Aotearoa Patient Safety Day screensavers, suggested social media posts and images, and newsletter content for organisations to use if they wish.

Do you have an example of a relevant quality improvement initiative?

We are also interested in sharing examples of quality improvement initiatives with a focus on reducing medication harm in transitions of care. If you have a story to share, please contact with a brief description and we will contact you to follow up.
