National review of avoidable deaths

Our role is to reduce avoidable deaths in New Zealand. We examine what contributes to a person’s death, and work with families and whānau, communities, clinicians, health services and other parts of government to prevent avoidable deaths.

We analyse death-related data, a person’s interaction with health and community services, listen to stories from family and whānau, and make recommendations to help our society understand and take action to reduce avoidable deaths.
