This document is a copy of the adult hospital inpatient experience survey questionnaire.
A maternity-specific module was added in May 2025 to provide more information about the experiences of people receiving hospital maternity inpatient care.
Questions that apply exclusively to the maternity cohort are clearly marked throughout.
You can also view a test link to go through the survey online just like a (non-maternity) patient would when they receive an invitation email
