The Choosing Wisely campaign seeks to reduce harm from unnecessary and low-value tests and treatment.

Your valuable feedback will be used to help improve the quality of care and patient safety. Your feedback has the power to change how health services are provided to you, your whānau and your community in the future. 

Taking part is voluntary and will not affect access to health care by you or your whānau. This survey is an opportunity to anonymously tell your hospital what they did well and what they could do better. Your answers will not be connected to you in any way. We value your feedback.
 
The survey should take about 10 to 15 minutes to complete, depending on your answers. Once you start the online questionnaire, you can return later to complete it.
