This page is for anyone who has received an invitation to take part in one of our patient experience surveys. Patient feedback can shape the health system and the future of health care. Your views are important to us.
Please help your health care provider understand what went well and if there is anything they could have done better by doing this 10- to 15-minute survey about your recent health care experience.
Your valuable feedback will be used to help improve the quality of care, patient safety and access to health services.
Your answers will be anonymous and will not be connected to you in any way.
