The shared goals of care programme is part of the patient deterioration programme at the Health Quality & Safety Commission. Shared goals of care are when clinicians, patients, families and whānau explore the patients’ values, the care and treatment options available and agree the goal of care for the current admission and if the patient deteriorates.
The Serious Illness Conversation Guide (SICG) is an ideal tool to support these discussions and the shared goals of care forms include the SICG questions and place to capture the answers.
Shared goals of care form links can be found in the talkingCOVID section of our website (but are not limited to COVID-related conversations only).
TalkingCOVID was created to help clinicians navigate care planning and decision-making during COVID-19 in an empathetic and person-centric way.
There are some great additional training resources for serious illness conversations as well as the shared goals of care forms and fact sheets.
The Commission has created a video to support you to feel more prepared when talking to patients/residents and whānau about their prognosis, their priorities, concerns and goals of care.
The training video will help clinicians in all health settings. It includes information about the guide, the adaptation of the guide to support conversations about COVID-19 prognosis and goals of care.
