Interventions

The Commission rolled out surgical teamwork and communication interventions to district health boards between August 2015 and April 2016.

The interventions were:

• briefing before the list begins
• paperless surgical safety checklist during each operation
• debriefing at the end of the list
• supporting communication tools:
  • closed-loop communication
  • call-outs
  • two challenge rule
  • ISBAR (identify, situation, background, assessment, recommendation).
