Aged residential care analysis of harm (adverse) events 2024/2025

Aged Residential Care (ARC) providers have completed their first full year of reporting Severity assessment codes (SAC) 1 and 2 clinical harm events through a single national reporting pathway.

From 1 July 2024 to 30 June 2025, ARC providers reported 2,004 events to the Commission . 

Falls were the most common type of harm reported by ARC providers (1,154 SAC 1 and 2 events), followed by clinical management harms. Most clinical management harms were pressure injuries (748 events). 

We acknowledge the hard work of ARC providers to adopt this new process of reporting to the Commission. 

Analysis of ARC’s first year of reporting shows opportunities to strengthen system‑focussed learning, rather than focusing on individual actions or education.
