Harm (adverse) events in health care have significant impacts on consumers, whānau, health workers and communities. Each event also represents an opportunity to heal, learn and improve how the system supports safe care.
This infographic presents a summary of serious harm events reported across New Zealand's health and disability sector during 2024/25.
The harm reports enable us to identify local, regional and national opportunities to support a culture of system learning.
The Commission combines harm reporting with other data sources to identify system learning opportunities.
