2024/25 Health and disability sector harm (adverse) events

Harm (adverse) events in health care have significant impacts on consumers, whānau, health workers and communities. Each event also represents an opportunity to heal, learn and improve how the system supports safe care.

Reporting for the 2024/25 year reflects expanded sector participation in reporting with aged residential care reporting their first full year.

The reports enable us to identify local, regional and national opportunities to support a culture of system learning.

The Commission combines harm reporting with other data sources to identify system learning opportunities.
